RESEARCH ON MEDICINE PRESCRIPTIONS AND DISPENSING OF MEDICINES USING ELECTRONIC PRESCRIPTIONS FOR PATIENTS WITH MENTAL DISORDERS

Abstract

The study is devoted to the analysis of the structure of medical prescriptions and dispensing of medicines according to electronic prescriptions for patients with mental disorders within the framework of the state health care guarantee program (hereinafter referred to as the health guarantee program), in particular the reimbursement program.During the study, it was found that the total number of doctors by specialty who can prescribe prescriptions under the reimbursement program in the period from 2021 to 2024 was from 34 695 to 37 325 providers, the number of electronic prescriptions created in 2024 amounted to 362 355 records, the percentage of prescriptions paid by pharmacies was 89,9% of the total number of prescriptions issued. At the same time, the number of doctors providing specialized medical care is significantly lower, therefore the need to involve primary care providers is a priority task to ensure patient access to such care. An analysis of prescriptions issued was also conducted for three areas of financing with a distribution by region, namely, by the medical guarantee program, the volume of patient funds and expenditures from the local budget. At the same time, a detailed analysis of groups of international non-proprietary names (INN) for medicines (MEDs) allows us to track the structure of prescriptions for each group of conditions (nosologies) of patients with mental disorders (MDs).The results of the analysis indicate the importance of expanding the medical guarantee program for patients with MDs.According to the results of a comparative analysis of issued and redeemed electronic prescriptions with a distribution by region, there is a trend towards a significant decrease in redeemed prescriptions that patients purchase at their own expense, which indicates an increase in out-of-pocket expenses for citizens, as well as the need to monitor and assess the effectiveness of the reimbursement program to increase patient access to MEDs.
